Initial batches of COVID-19 vaccines are being disbursed across the nation. Women — who make up the majority of health care workers, as well as a disproportionate number of older Americans and frontline workers — are at the front of the queue for immunizations.

But the nation’s vaccine distribution process has been slow and disjointed, prompting worries about whether the government has adequately invested in delivering the new injections to people in need.

We want to hear from you. Have you received a COVID-19 vaccine, or are you scheduled to get one soon? What has the process been like? Based on your answers to the questionnaire below, we may follow up with you for a story.
